Contribution to Undergraduate Program
INTRODUCTION
The aim of the undergraduate course in obstetrics & gynaecology of the University of Rajarata, Sri Lanka is to provide training in the main stream of surgery to undergraduates. It is a skills development course. The students are expected to acquire skills of history taking and examination to arrive at a basic clinical diagnosis, order and interpret relevant investigations & formulate a plan of management. They should establish a good rapport with the staff in addition to development of good discipline and attitudes. The course is aimed at helping the undergraduates to develop skills knowledge and attitudes in the discipline of surgery enabling him to function as a doctor of the future.
The students in the Faculty of Medicine and Allied Sciences in the University of Rajarata learn Obstetrics and Gynaecology starting from third year through to the fifth year. The priority of department is the provision of high quality and innovative teaching and guidance to the undergraduate to develop knowledge, skills and attitudes in the discipline of obstetrics and gynaecology to the highest standard enabling them to function as a good clinician in the future.
ORGANISATION OF OBSTETRICS AND GYNAECOLOGY COURSE
The main areas in the program include:
- Obstetrics and Gynaecology lectures
- Obstetrics and Gynaecology tutorials
- Clinical skills training
- Regular ward classes
- Seminars/workshops recommended by the department
- Regular in-course and final assessment on completion of the course
OBSTETRICS AND GYNAECOLOGY LECTURES
Lectures are conducted to teach the theoretical aspect in Obstetric and Gynaecology and will help students to understand patho-physiology of pregnancy and its complications and also Gynaecology. Basic knowledge given by the lectures will help students to understand clinical conditions better and help to assimilate it in their clinical practice.
The Obstetric and Gynaecology lecture schedule is designed to cover most important and common problems encountered in Obstetric and Gynaecology, but students are expected to read and gather information from sources when required. The timing and venue of lectures will be notified by the clinical coordinator or relevant authority.
The basic structure of a lecture by the Department of Obstetrics and Gynaecology generally should/may include following areas of a condition.
- General introduction – optional
- Epidemiology
- Aetiology
- Pathology/pathophysiology/natural history & progression of the disease
- Clinical features
- Investigations Management – treatment, advice, follow up etc;
- Prognostication – optional
- Screening – optional
Though this is the common format, a lecture may be structured based on signs & symptoms of clinical presentation, on principles of Obstetrics and Gynaecology or any other structure that cannot be fitted into the above generalized structure.
In addition to departmental lectures there will be lectures delivered by visiting and guest lecturers. The Obstetrics and Gynaecology department may notify about these lectures and will recommend students to attend them. In addition to these, Obstetrics and Gynaecology department undergraduates are advised to attend medical/clinical lectures or presentations conducted by other bodies to update their knowledge.

CLINICAL TRAINING
The clinical training in obstetrics and gynaecology is scheduled via two non professorial and one professorial appointments.The compositions of two non professorial appointments are summarized and tabulated as follows

The professorial appointment will be done in the final year after completion of all above appointments and successful completion of para-clinical courses.
Professorial Obstetrics and Gynaecology appointment consists of 8 weeks of detailed clinical work done in the university Obstetrics and Gynaecology unit.
GENERAL GUIDELINES FOR CLINICAL APPOINTMENTS
The students must adhere to the guidelines given below for the successful completion of the obstetrics and gynaecology course in the Faculty of Medicine, University of Rajarata. If it is not, they won’t be eligible to sit for the final MBBS examination.It is student’s responsibility to be with their programmed obstetrics and gynaecology course and discuss the inadequacies with the relevant authorities.
- Obstetrics and Gynaecology appointments will be held at the Teaching Hospital Anuradhapura.
- Students must always dress properly. When attending clinical works, a white coat with the name tag should be worn.
- Students must carry basic equipment such as stethoscope, torch, measuring tape and a note book with them. Borrowing is not recommended.
- Students must be punctual and present at the relevant location at leastby the required time orÂÂÂÂÂ as directed by the supervisor and should not leave the premises without permission from the supervisor or his/her delegate.
- 100% attendance is compulsory unless a written acceptable reason for absence is provided..If a student’s attendance is insufficient, the supervisor will decide the necessity to repeat the appointment or part of it depending on his/her competency after assessment.
- If a student is absent for three or more day’s he/she must submit a medical certificate.
- Maintenance of discipline after graduation is the corner stone to practice as a good doctor. Hence good discipline is a basic requirement for medical students and all should take initiatives to improve discipline.
- The key to good clinical practice and subsequent patient care depends on establishing good rapport with the patient. Furthermore students must respect the patient’s autonomy (right to refuse to be examined by student), privacy and should always maintain confidentiality with regards to information provided by the patient.
- Students should always carry the log book with them and should provide it If the consultant, SR, R, SHO or HO’ request for
- Students should fulfill the tasks which are given in the log book as much as possible. At the end of professorial appointment the log book have to be handed over to the department. Marks will be awarded for the content and it will be incorporated in to the final MBBS marks under continuous assessment.
- For each patient in the ward/Clinic, there should be two owners and they should be ready with histories which could be randomly checked by senior staff.
- On casualty admission days, the relevant groups of students are expected to stay in the casualty ward/Theatre as decided by the responsible consultant in the Department of Obstetrics and Gynaecology. During post casualty ward rounds students are expected to know about all the patients in the ward.
- During the theater sessions, students should follow the theatre rules. It is a must that student should adhere to sterility protocols maintained by the Obstetrics and Gynaecology theatres and other relevant places & owner of the patient should know the patient and should assist or observe the prosedure.
At the end of each appointment, there will be an evaluation on the knowledge, skills and attitudes.
